          o    Provides patient advocacy and care coordination for individuals managing chronic illness, complex healthcare needs, and social barriers impacting access to care. 
          o    Helps clients navigate healthcare systems and access supportive community resources that address social determinants affecting overall health and well-being.
          o    Advocates for individuals experiencing healthcare access challenges by coordinating resources, provider communication, and supportive services. 
          o    Supports patients and caregivers through healthcare navigation, community resource connection, and coordination of services addressing social and environmental needs.
